Cohort Review Template
The primary focus area of this tool is Cohort Review; it relates to multiple NTIP focus areas. This tool is a template to be used by presenters in preparation for an upcoming Cohort Review. Data are collected on the form in a concise format that can then be presented during a Review. This tool has been used and revised by the Utah Department of Health since March 2003 when Cohort Reviews were first implemented in the state. Active TB patients are the primary targets for the tool. Lastly, this tool requires both quantitative and qualitative analysis. Questions regarding this tool may be referred to Larry Niler at lniler@utah.gov.

Cohort Review Tool: PDF Version Word Version
This is a cohort review tool which addresses Completion of Treatment within 12 months, Sputum Culture conversion, known HIV status and Contact investigation outcomes. The tool addresses the following questions: Did the patient complete treatment within 12 months? Did the patient have documented sputum culture conversion within 60 days? Was the HIV status known? Were contacts identified and evaluated? Did contacts start and complete treatment? The tool requires quantitative analysis, and no additional training is needed. The tool has been used 13 times for county level cohort reviews in NYS and 2 times for regional reviews. Tennessee
Cohort Review Form
This is a cohort review tool which addresses treatment completion, contact investigation outcomes, treatment initiation, sputum conversion, drug regimen, genotyping, HIV status, and reporting of culture results. This tool is used for counted cases within a given quarter. Tennessee has used this tool during five (5) cohorts that we have done across the state, and is constantly receiving input from field staff on this worksheet. A revision will be conducted after the final cohort review of the year. This tool requires quantitative analysis, and the state has a cohort review worksheet data dictionary (that is currently under revision) and has held face-to-face trainings with field staff on how to use this.
